The Thrive Program at The PAC with JD Health and Wellness is a comprehensive support system for young athletes, created in partnership with schools, health providers and insurance companies to ensure every participant’s holistic well-being without any financial barriers. Our program goes beyond athletic training to include academic support, life skills development, and mental health resources, helping young athletes thrive as balanced individuals.

 

Key features include:

  • Sports Development: Focused training in sports fundamentals and performance accountability encourages athletes to pursue excellence through clear goals.

  • Academic Support: We offer tutoring and guidance to help athletes succeed in the classroom, reinforcing the importance of education in achieving their long-term goals.

  • Mental Health & Wellness: In collaboration with health providers, we prioritize mental well-being, offering resources and peer support to help athletes manage stress and cultivate a positive mindset.

  • Workforce Training:  Through partnerships with local businesses and health providers, we offer workforce training that equips athletes with valuable job skills, including time management, communication, and teamwork. Our program also provides hands-on work experience and internships, helping participants build a foundation for future careers both within and beyond sports.

Step 1: Contact Us & Apply for Scholarships
Reach out to our team and fill out the scholarship form to check eligibility. If you qualify, we can offer access to Thrive programs, including sports, tutoring, and much more!
 

Step 2: Insurance Verification
If your child has PacificSource DMap insurance, we’ll confirm eligibility and move forward to schedule their next step. (More insurance will be coming on board soon).


Step 3: Health Intake Appointment


Next, we’ll schedule a health intake with a licensed professional. This intake ensures your child receives personalized care, including physical health, mental wellness, and workforce development support.


Step 4: Meet Your Community Health Worker (CHW)
Your child will be paired with a dedicated Community Health Worker who will help them navigate through our services and track their progress.


Step 5: Access to All Services
Once the intake is complete, your child gains access to all of our resources—physical activities, mental health support, academic tutoring, workforce development, and more.


Step 6: Follow-Up & Continuous Support
Your CHW will regularly follow up to ensure your child is benefiting from the programs, growing mentally, physically, and gaining valuable life experience.
